Join us for a free, open house-style Advanced Directive Workshop, where Oaklawn Case Managers will be available to: Provide the necessary advanced directive paperwork*, answer questions about the process and options, assist you in completing the paperwork on-site, or offer copies for you to take home and fill out at your own pace

Drop in anytime during the event—no appointments or registration required. Whether you’re just starting to think about an advanced directive or ready to complete the forms, we’re here to support you in taking this important step.

*Advanced directive paperwork is a legal document that allows individuals to communicate their wishes regarding medical care in advance, especially when they are unable to do so themselves due to illness or injury. These directives can specify which treatments a person wants or doesn’t want, and can also appoint a healthcare proxy to make decisions on their behalf.
