4 Golfers per Team
You may sign up as a team or as an individual.

$5,000 – Executive Sponsor: title sponsor, with banner at clubhouse. Includes foursome for tournament.

$1,500 – Cart Fleet Sponsor: name on all golf carts in fleet. Includes foursome for tournament.

$1,000 – Lunch Sponsor: recognition at lunch.

$500 – Scoreboard Sponsor: name or logo on scoreboard

$500 – Putting Green Sponsor

$250 – Hole Sponsor: hole sign on course

$200 – Breakfast Sponsor: signage at breakfast

$100 – Friends of Keri

Breakfast & Lunch Only (non-golfers): $35

Here are the steps to register or sponsor:

1) Click the link to get to the event home page:  http://causeforeboz.anniesangels.org)

2) To register or sponsor, select "TICKETS" at the top of the page (if you wish to make a donation of a specific amount and not be a sponsor, select "DONATE")

3) Under "Purchase Tickets" , the Player Entry Fee and the various Sponsor Levels to choose from will be listed.

4) Find what you want to purchase, hit the + sign to select how many "tickets" you wish to purchase.  If registering a foursome, select 4 under Player Entry Fee.

5) On the right side, you will see "Order Summary", listing out your selections.
*Note - Annie's Angels has added a new default "Tip" to the order, to help offset transaction fees.  This is optional, and you have the choice to remove, increase or decrease this amount..  To opt out of the "tip" and only pay the registration or sponsor amount, simply hit "Set custom amount" and the tip will be removed.

6) When your order amounts are set, continue to fill out the rest of the form - Name, Email, Team Name (teams of four only), Player Names, Email Addresses, Phone Numbers (optional).  Select "Continue".

7) Enter Payment information, select "Pay".
