Pre-registration is required and must be completed a week prior to the clinic. No walk-ins accepted. Pre-register at https://form.jotform.com/252595968000159.

By completing the pre-registration form, parent(s)/guardian(s) are giving consent to LCHD to vaccinate their child with the flu and/or Tdap vaccine while at school. Parent(s)/guardian(s) do not need to be present at school based clinics. Please have your child dress appropriately so their upper arm is easily accessible the day of clinic.
