Winter Dance Fitness Class

My goal in teaching dance fitness classes is to make it as accessible to everyone as possible while getting a great workout in! This class will have a mix of easy to follow dance moves and exercises (squats, lunges, etc.) bring a towel, water and a smile! As long as your feet stay moving that's all that matters!

*About the location: when you arrive please go to the far back green awning . Though the glass door theres a staircase immed to the right. Please read this entire document carefully before signing. By signing below, you are forever giving up certain legal rights. This waiver permanently releases THE PARTICIPANT from liability resulting from your participation in Dance Fitness with Alyssa activities.

In consideration of being allowed to participate in Dance Fitness with Alyssa activities, by signing below, I (participant) hereby agree as follows:

I am 18 years old, or older. This waiver binds the members of my family and spouse, and my estate, heirs, administrators, personal representatives, assigns, and any other person entitled to act on my behalf.

Participants also acknowledge the risks involved in Dance Fitness with Alyssa. These risks include, but are not limited to: intense movement, dehydration, fainting, soreness, pulled muscles, infections due to communicable pathogens, abrasions, broken bones, seizures, or other physical injuries. Participant states that Participant’s participation in Dance Fitness with Alyssa is voluntary. Additionally, Participant states that Participant does not have any conditions that will increase the likelihood of experiencing injuries while engaging in this activity. I acknowledge that I have consulted with a physician and I am physically able to participate in Dance Fitness with Alyssa activities.

Participant will also make every effort to obey safety precautions as listed in writing and as explained to participant verbally. Participant will ask for clarification when needed.

I acknowledge that Dance Fitness with Alyssa activities might occur on Dance2Fit property, or at other locations, and I hereby voluntarily accept the risks associated with Dance2Fit activities. I agree that the Dance Fitness with Alyssa will not be responsible or liable for any personal injury, including death, regardless of the cause of the injury or death. I hereby acknowledge that Dance Fitness with Alyssa is not responsible for any claims, damages, losses, or costs that might arise due to my participation in Dance Fitness with Alyssa.

The undersigned, while participating in any such activities, including anyone entitled to act on his/her behalf, hereby releases, and agrees to indemnify and save harmless, Dance Fitness with Alyssa, its successors, assignees, officers, agents, and employees, from any and all claims, demands and causes of action in anyway growing out of or resulting from Participant’s participation in the activities.

Participant irrevocably forfeits all right to bring a lawsuit against Dance Fitness with Alyssa for any reason.

This Agreement will be construed under the laws of the State of Ohio without regard to its conflict of law provisions. If any portion of this is held to be invalid, illegal, or unenforceable, the remaining portion shall be in full force and effect.
