I acknowledge that I have consulted with a physician and I am physically able to participate in Dance Fitness with Alyssa activities.Participant will also make every effort to obey safety precautions as listed in writing and as explained to participant verbally. Participant will ask for clarification when needed.I acknowledge that Dance Fitness with Alyssa activities might occur on Dance2Fit property, or at other locations, and I hereby voluntarily accept the risks associated with Dance2Fit activities. I agree that the Dance Fitness with Alyssa will not be responsible or liable for any personal injury, including death, regardless of the cause of the injury or death.
