Eye Exams (CAER):

• Must pre-register by Monday May 4th***
• Minimum age for examination is eight weeks.
• Arrive 15 minutes prior to exam time for eye drops
• Please register at OFA website and bring OFA printed application
forms with you to the clinic (instructions below)
o Log on to online.ofa.org
o Select “Create a New Application”
o Fill out your dog’s information
o (the search function will populate all the data if dog is already in the OFA system)
o Choose “Online Exams”
o Scroll down and click on “Sun May 17 - Suffield CT - CAER Eye Exam”
• Paperwork must be completed online prior to the clinic.
• AKC registration & permanent identification is required for OFA.
• Registration & payment must be sent to CRVGRC otherwise you
will not be on our schedule. Registration form available at
CRVGRC.org
Heart Exams:
Dr. Lauren E. Wiley VMD, ACVIM- Cardiology
Basic cardiac (auscultation only) Fee $65
Advanced Cardiac (auscultation + echocardiogram/doppler) Fee $300
• Must pre-register by Monday May 4th***
• Initial screenings only.
• Goldens will get preference if registered by April 6th
• Registration form available at CRVGRC.org
• Please provide dog’s age.
XRAYS - Hips & Elbows:
Dr. Ann Huntington, DVM
Fee $300
• Must pre-register by Monday May 4th***
• Goldens will get preference if registered by APRIL 6th
• Contact us for weight restrictions before registering your large
breed dog.
• A completed OFA Form MUST accompany the CRVGRC
registration form even if not submitting to OFA.
• OFA Fee paid separately, please send in with this form. Credit
card preferred.

Microchip:
Fee $40
