Antenatal Exercise Medical Clearance Form

*Patients who do not have a Medicare number will be charged $339.00 per appointment. Please contact the cashiers office on (08) 6457 8217 and/or your Insurance Company for any queries.

I f you have a current Medicare card - this class will be free, please note if you are asked for payment please contact North Metropolitan Health Services - (08) 6457 7991 and report any suspicious activity.

Wear comfy clothes, closed in shoes and have a water bottle ready.

When you attend your first class, you will need to bring a consent form signed by your doctor or midwife stating that your pregnancy is low risk and it is safe for you to exercise. Copies of this form are available in the Antenatal Clinics, Family Birth Centre and from your KEMH/OPH Women's Health Physiotherapist or the link above.

If you have any difficulty booking into the class please call before 3pm Wednesday.

PLEASE NOTE: Participation is not allowed if you don't bring this completed form to your first class. You must bring a valid Eventbrite confirmation ticket to every class which must be shown at reception for entry.
