Live & Die Well Club: POST Forms

Our new Live and Die Well Club will meet monthly, with the goal of improving local palliative and hospice support and normalizing discussions about death and dying. At our first meeting, learn about POST forms (Physician Orders for Scope of Treatment) and how to file them properly, and maybe even fill one out. POST forms help people communicate their treatment preferences to healthcare providers. They ensure that preferences are honored regardless of your location or ability to communicate. We'll also talk about Healthcare Power of Attorney forms and the importance of talking with loved ones about your wishes. We appreciate Physician Assistant Amy Klingler for helping lead this program.
