Sequential Intercept Model mapping helps communities develop and implement plans for community change through cross-system collaboration, utilizing innovative and dynamic tools to map systems, identify gaps in service, and clarify community resources and opportunities.The goals of the workshop are:Facilitate mutual understanding, collaboration and relationship building between a diverse array of stakeholders, all of whom are dedicated to system transformation and committed to remain connected and committed to achieving common goals.Identify best practices, resources, gaps in services, and opportunities for improvement and innovation across all SIM intercepts.Prioritize key steps toward system transformation and improved service delivery.Create a longer term strategic action plan, optimizing use of and increasing the availability of local resources in Hays County and furthering the delivery of appropriate services.This intensive, interactive workshop will result in a Sequential Intercept Map of Hays County and action plans that will inform community priorities and direct local action.Our process includes two meetings.
