The Summit will offer participants valuable insights on driving advancement in the field of Health Care Transition (HCT). Participants can use this knowledge to enhance their clinics, hospitals, programs, and overall state practices to support safe transition from pediatric to adult care systems.We are delighted to welcome Dr. Patience White and Peggy McManus, from Got Transition, a program of the National Alliance to Advance Adolescent Health.
