Sports Physicals at Bullhook Community Health Center

Join us for a one-day Mobile Unit Sports Physical event at Bullhook Community Health Center on Wednesday, August 6 from 7:30 AM to 6 PM.

This pop-up clinic in our parking lot offers quick, walk-in sports physicals for middle and high school students. No appointment is needed.

Cost: $49 (cash, check, or credit card)
Location: Bullhook Community Health Center, 521 4th Street, Havre

Please bring:
• Completed sports physical packet (available at Bullhook or online at https://shorturl.at/GSoYs)
• Insurance card
• A parent or guardian if the athlete is under 18

Packets can be dropped off at Bullhook ahead of time. Stop by and get your student ready for the fall sports season!
