Planning for the Future - Bullhead City

“Planning for the Future” workshop attendees will receive the Arizona Advance Directive (for health care) forms with instructions on how to complete them. Additionally, everyone will receive the “What My Family Needs to Know” workbook to help identify and organize important information, documents and accounts.
