Hominy JOM School Supply Distribution

The parent committee will distribute school supplies, for Native American students enrolled at Hominy school district, on Thursday, August 7 from 6:00 β 7:30 p.m., at the Osage Nation Education Department (ONED). ONED is located approximately three miles North of Hominy in the Osage Industrial Park. The address is 102 Buffalo Avenue, Hominy, and the building is brown rock with a tan roof. School starts on Thursday, August 14.
Parents of new students need to submit a copy of their student's tribal membership card, verification of enrollment, and JOM student form application to Avis Ballard, JOM Coordinator, electronically (YWJhbGxhcmQgfCBvc2FnZW5hdGlvbi1uc24gISBnb3Y=) or through mail (PO Box 250 in Hominy 74035). A parent's or grandparent's tribal membership card/document will also be accepted. The student's and/or parent's birth certificate will need to be included in order to verify lineage.

The JOM Needs Assessment Survey is now online. This annual assessment is conducted every year. Please take this survey if you have not done so yet. Each parent or guardian can complete it. One survey per household is required for Johnson OβMalley (JOM) services, including school supplies. Completed questionnaires will be sent to the JOM coordinator's email address.
