Lighthouse End-of-Life Planning Program with St. Croix Hospice

While we can’t predict the future, we can always create a plan to support our loved ones ahead of time. Join us as St. Croix Hospice presents their Lighthouse End-of-Life Planning Program! Having a plan documented can light the way for your loved ones after you are gone. This program provides complimentary tools (including Power of Attorney forms), information and training to help you plan for the future.

Anyone age 18 or older should put a plan in place. These resources will help your providers and loved ones make informed decisions based on your wishes. Be in control of your future and light the way for those you love.
