Lakeview PATH

Assistance in Transition from Homelessness (PATH) is designed to help people struggling with mental health issues to obtain housing and stability. PATH links eligible people to mental health and substance abuse services, including applying for Supplemental Nutrition Assistance Program (SNAP) benefits or Social Security Benefits (SSI/SSDI).

Available 1st and 3rd Tuesday each month
