Emmanuel Project is joining Community Church of Keystone Heights for the third year and during both Thursday 5p-8p & Friday 9a-12p school shopping events.
Back-to-School physicals and sports physicals are being provided FREE for children without health insurance or lack access to healthcare.
The required school DH3040 physical Exam & certified 680 vaccine forms will be provided. Forms provided the same day for scheduled appointments.
