You always have the right to make your own health care decisions—but what if illness or mental incapacity takes that ability away?

Who will make decisions on your behalf? Will your wishes be known and respected? Who Will Speak for You If You Can’t Speak for Yourself?

It’s important to have “the conversation” with your family and friends, and even more important to put your wishes in writing. An advance directive ensures that your voice is heard—even when you can’t speak. It provides clarity and peace of mind for you and
your loved ones in a time of uncertainty.

Following the training, and as time and interest allow, Jennifer Haase, MAC, SW, is available to help you complete a Power of Attorney for Health Care.
