Tools for School and Lace Up for Kids Applications

Applications will be accepted at Eastside Community Ministry (In person ONLY) Monday, July 7 - Thursday, July 10 from 9am-4pm for Tools for School and Lace Up for Kids.
Tools for School provides backpacks filled with supplies for students in grades 1st – 12th attending schools in Muskingum County. You must have the following information with you when you are completing your application:
• ID for parent/guardian completing application,
• Identification for the child (Birth Certificate, SS Card, or Medical Card)
• Proof of Address
Lace Up for Kids provides tennis shoes, socks, new outfit, underwear & a health fair for students in Muskingum County School District in grades Pre-K - 12th.
