Using digital systems to detect and manage patients for CKD, 23 April | Online Event | AllEvents.in About the event Using digital systems to detect and manage patients for CKD
A series of webinars about CKD and the resources available to help you to improve outcomes for people with CKD.

About this Event

As part of the wider Cardiovascular Disease Prevention Programme, HI NENC is delivering a Chronic Kidney Disease (CKD) programme Chronic Kidney Disease - Health Innovation NENC. The CKD programme will focus on improving earlier diagnosis and supporting early medical treatment of people with CKD to slow disease progression and reduce the development of cardiovascular comorbidity. Join us to learn more about CKD and the resources available to help.

Through facts.CKD programme HI NENC will support Primary Care Networks (PCNs) to implement the North East and North Cumbria renal Operational Delivery Network Guidance for CKD and improve diagnosis rates in people with CKD.

A key priority of the programme is to reduce health inequalities; we know that approximately 50% of coded CKD resides in 20% of practices within the areas of highest socio-economic deprivation. We aim to work with our partners, including the North East and North Cumbria Integrated Care System (ICS) to ensure that activities are focused on areas of higher deprivation and groups of people who are at higher risk of developing cardiovascular disease as a result of their CKD.

This webinar is the second in a series of three webinars for all staff working in Primary Care Networks (PCNs) including Nurse practitioners, ARRS roles/Pharmacists and GP's. Throughout the series, attendees can expect to learn more about CKD and the resources available to help you to improve outcomes for people with CKD.

We will focus on how to ‘Detect’ patients for CKD with the greatest need for intervention, monitoring or referral.

Given the high prevalence and high comorbidity profile of CKD the webinar will share tips on how to use CDRC for CKD – (Clinical Digital Resource Collaborative) to effectively manage the CKD population in a way that supports achievement of other Cardiovascular QoF targets.

The webinar will also support HCP’s in understanding which patients to refer to Renal services and how to do this using easy to access validated risk assessment tools.

About The Host:
The Academic Health Science Network for the North East and North Cumbria (AHSN NENC) is dedicated to improving healthcare and supporting wealth creation through partnership working and promotion of innovation across the NHS, Academia and Industry. With a remit to provide system-wide integration, we facilitate interactions with multiple partner organisations across the region. Principally we work with the NHS Trusts, CCGs and Universities which form our core membership, and we collaborate closely with many other stakeholder groups. We support the inclusive, collegiate approaches that are needed to address shared problems at a system level. Wealth Creation and Health Improvement have a common requirement for innovation. The AHSN NENC supports all aspects of innovation across healthcare from creating a culture that supports the innovation through to the dissemination and adoption of effective practice and proven technologies. We have therefore achieved the regional reach and the local buy-in that is necessary to deliver cross-organisational programmes to improve health and create wealth in the region.
