Generation of Excellence Trendsetters Nonprofit is proud to announce that we are the first organization in the nation to solidify a partnership with Dove Hydration and Wellness, Aja Healing, and Maskia Health Wellness Clinic under the new Community Health Integration (CHI) Medicare Program.
Through this innovative collaboration, Community Health Workers (CHWs) will have the opportunity to address Social Determinants of Health (SDOH) for patients at each partner clinic through a custom, HIPAA-regulated referral system. This system will strengthen care coordination, enhance access to resources, and improve overall patient outcomes.
