Congressman Ellzey from the 6th congressional district of Texas is hosting a Passport Fair with the Dallas Passport Office to assist with applications and renewals here at the Midlothian Public Library! This event will take place in the Community Room of the Library + City Hall.

Requirements: 
All persons applying for a passport must register and be present at the event.
For information about forms, special requirements for minors and photo requirements, please visit the Department of State website.
Please bring certified copies of all documents including your valid driver’s license, plus one of the following as your proof of U.S. citizenship:
A previously issued U.S. passport
original birth certificate,
or certificate of naturalization.
Each applicant must bring a front and back photocopy of the ID that will be presented at the time of passport acceptance. Local university ID is acceptable for college students.
Applicants are required to provide one recent passport photograph, measuring 2-by-2 inches, on a white background. Applicants may purchase photographs at a local CVS, FedEx, Walgreens, etc in advance of the fair. The Midlothian Public Library is collaborating with a local photographer to help provide required photographs for free on the day of the event.  
All applicants must appear in person (including minors). Minors under the age of 16 must appear with both parents. If one parent is not available, a notarized DS-3053 Statement of Consent authorization from the non-appearing parent is required, along with a photocopy (front & back) of the absent parent’s ID. U.S. passport holders whose passport expires within six months should consider renewing their passports.
