Who would you trust to speak your treatment preferences to your medical team?  Does that person know your preferences? Led by fellowship member Amy Bailey.
Continue the Conversation After the Service: After worship, fellowship member Amy Bailey will be available in the library to answer questions and help you review your advance care planning documents, including advance directives, durable powers of attorney for health care, and other related paperwork.

If you already have these documents, feel free to bring them along. Amy can help you understand what they say and discuss any questions you may have. Don't have them yet? Blank forms will be available to help you get started.

UUFoM will also have Life Crisis Forms available for you to complete. It's an easy way to organize important information for your family and caregivers. No appointment needed. Just stop by after the service.
