Breaths Over Troubled Waters: CHF & COPD in Long Term Care


Join Nurse-Led Outreach Teams (NLOTs), long term care home administrators and staff, community paramedics, Behavioural Supports Ontario, LHIN administrators, and others for a day of learning and sharing to increase awareness and understanding of CHF and COPD in long term care settings.

Presenters include:

KEYNOTE: Dr. George Heckman from the University of Waterloo - leading expert on the management of heart failure in the frail elderly in long term care homes.

Morgan Krauter, NP at Southlake's Heart Function Clinic, and Michelle Wilson, NP & NLOT nurse at Southlake will present on the use of OTN for follow-up with residents in long term care.

Dr. Leah Steinberg, MD with the Temmy Latner Centre for Palliative Care will talk on the challenges and practices of providing palliative care to end-stage heart failure patients.

Wendy Cheung, RN and Director Medicine Program at North York General Hospital will review CHF/COPD Clinical Care Pathways and the group will explore how they can be applied to a long term care setting.
