An Introduction to Transition from Children to Adult Hospice Services.

Learning outcomes:

• Understand that is meant by transition, both locally and nationally
• Have an overview of the differences and similarities between children and adult hospice care
• Be aware of the impact of transition on young people , families / cares and staff
• Understand the needs of young people
• An introduction to legal aspects of care for young people
• Understand the multidisciplinary approach to transition
• Gain confidence to support young people more effectively in an adult hospice setting
