BIRTH CERTIFICATES OR SS CARDS FOR THE CHILDREN
PROOF OF ALL HOUSEHOLD INCOME FOR THE PAST 30 DAYS OR
A VALID DHS AWARD LETTER WITH THE CURRENT DATES
