About the event

Planning for the Future
In collaboration with Lake Havasu Senior Center, Hospice of Havasu is presenting the Planning for the Future workshop on Thursday, July 17th from 9AM – 10AM. RSVP 928-453-2111.

Advance Care Planning is important for everyone, regardless of age. Letting others know of one’s healthcare preferences is essential in the event of incapacitation resulting from illness or injury. This workshop provides and reviews the Arizona Advance Directives and discusses other important conversations about “What to do When..."
Advance Directives include:
• Health Care Power of Attorney
• Mental Health Care Power of Attorney
• Living Will
• DNR (Do Not Resuscitate)
• POLST (Physician Orders for Life Sustaining Treatment)
