OC Network of Care Convening

Please join us for the OC Network of Care Convening on Monday, November 17th from 2 p.m. to 4 p.m. at the Orange County United Way office. Our agenda will focus on Priority 2 (see below). We will also have updates from Orange County United Way, DxF Collaborative/Blue Path Health and Equity Table as well as continue our strategy work with Priority 3.

Priority 1: Coordinating Infrastructure for Community and Clinical Connections

Strategy: Promote an interoperable closed-loop referral ecosystem and community/health data information exchange to support care coordination across the Continuum of Care.

Priority 2: Convene and Align Multi-Sector Network Centering Opportunity for All and Community

Strategy: Create alignment across partners, programs, and initiatives, supporting community-informed systems change and community-led advocacy efforts for policy change.

Priority 3: Promote Trauma-Informed Community Awareness and Workforce Education

Strategy: Increase the skills and confidence of the workforce to provide culturally responsive services that meet the needs of children and families, while building the leadership and capacity of the community.
