A DRIVER’S LICENSE OR ID
BIRTH CERTIFICATES OR SS CARDS FOR THE CHILDREN
PROOF OF ALL HOUSEHOLD INCOME FOR THE PAST 30 DAYS OR
A VALID DHS AWARD LETTER WITH THE CURRENT DATES
Screenshots of current DHS award letter with the effective dates may be presented at application.
Pay stubs or social security annual award letter
