SPEAKER:  Dr. Buck

Dr. Buck is an expert in community health, serving as a professor in the Department of Family and Community Medicine for more than two decades at Baylor College of Medicine in Houston, where he also received his medical degree. Dr. Buck holds adjunct professor positions at the University of Texas Health Science Center School of Public Health and Rice University. He also founded Healthcare for the Homeless-Houston and Patient Care Intervention Center to improve health care quality and costs for underserved populations.


PCIC has been developing and providing innovative approaches to Care Coordination, Population Health and Health Information Technology. Here is a look at PCIC as we evolved over the years, and the history of our parent organization – Healthcare for Special Populations.


PCIC's platform is designed to track, measure, and address complex determinants of health – including medical, housing and transportation, income and employment status, hunger and access to nutritious foods, literacy and language, behavioral health, social integration, and much more – by linking data from disparate medical and social provider systems to create a comprehensive record for each client.


PCIC is building a bridge between service providers to facilitate data sharing and cross-agency care coordination for Houston’s most vulnerable individuals. Through robust, cross-agency data analytics and patient care plans that are shared across agencies, our platform strengthens our region’s health safety net by empowering medical and social agencies to collaboratively address social and economic determinants of health.


The Unified Care Continuum Platform’s core functionalities include facilitating care coordination for patients with complex needs, linking need to resources in real-time, streamlining collaboration between medical and social service providers, and generating data to support advocacy for systems-level change.

Parking instructions below


Enter garage from Bertner Ave. Obtain a ticket from the parking machine as you take the ramp to the upper floors. Please park on the 2nd - 5th floors. Bring your ticket with you and we will validate your ticket.


We are on the 6th floor of the John P. McGovern Building. Once you get off the elevators the hostess stand is straight ahead and down the hallway.


To exit, circle back down to the 2nd floor. Insert your ticket in the payment machine on the driver’s side and exit to the right. Proceed down the ramp.


All are welcomed, but RSVP is required.
