Materials Provided:
Shadow box
Assorted pre-broken china plate and glass pieces (a variety of colors and patterns will be provided)
Misc decorations
Use of glue gun and other tools
Basic design template


Participant Requirements:
No need to bring any materials – everything is provided!
A willingness to learn and have fun!

Class Duration: 1-2  hours

**Due to limited seating and prep work that goes into these workshops, there will be no refunds issued on ticket purchases


RELEASE OF LIABILITY (to be signed at time of class)
I, the undersigned, hereby acknowledge and agree to the following release of liability for participation in a class offered by Clink Designs/Christina Link(hereinafter referred to as "the Instructor").
1. Nature of Activity: I understand that this class involves inherent risks, including but not limited to:
•	Working with glass, china/pottery, which can break and cause cuts, abrasions, or punctures.
•	Using tools including, but not limited to, glass cutters, mosaic nippers, grinders, saws, soldering irons, and pliers, which can cause injury if used improperly.
•	Exposure to potentially hazardous materials, including lead (in solder), and glass/china/pottery dust.
•	Potential for burns from hot soldering irons or glass/china.
•	Physical strain from repetitive motions.
•	Slips, trips, and falls within the studio environment.
2. Assumption of Risk: I understand and voluntarily assume all risks associated with participation in this stained glass class. I acknowledge that these risks may result in personal injury, property damage, or even death.
I agree to pay for any and all damages to the facilities, tools, supplies, and retail items by my negligent, reckless, or willing actions.
3. Safety Precautions: I acknowledge that the Instructor will provide instruction on safe techniques and procedures for working with glass and tools. I agree to:
•	Pay close attention to all instructions and demonstrations.
•	Follow all safety rules and guidelines provided by the Instructor.
•	Wear appropriate safety gear, including, but not limited to, safety glasses, gloves, that will be available.
•	Inform the Instructor of any medical conditions or concerns that may affect my ability to participate safely.
•	Use tools and materials only as instructed.
•	Take responsibility for my own actions and safety.
•	No food or beverage are permitted during class.
•	No one under 18 years old permitted without adult supervision*. *(Supervising adult must purchase additional ticket and sign release for minor.) 
4. Release of Liability: I hereby release, waive, discharge, and covenant not to sue the Instructor, Clink Designs/Christina Link, its owners, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to my participation in this class, including any injuries, damages, losses, or expenses incurred as a result of my participation, except in cases of gross negligence or willful misconduct on the part of the Instructor. Any legal or equitable claim that may arise from participation in the above shall be resolved under Maryland law.
5. Medical Treatment: I authorize the Instructor to obtain medical attention for me in the event of an emergency, at my own expense.
6. Photo/Video Release: I grant permission to Clink Designs/Christina Link to use photographs and/or videos of me taken during the class for promotional purposes, including but not limited to social media, website, and marketing materials.
7. Understanding and Agreement: I have carefully read this release of liability and fully understand its contents. I am agreeing to this document voluntarily and with full knowledge of the risks involved.
