Diabetes PATH: Personal Action Toward Health (Diabetes PATH) is a 6-week community diabetes education program to help individuals learn the tools and techniques needed for day-to-day management. The program was developed and rigorously tested by Stanford University.  

Benefits: Research studies found that after the workshop participants had significant improvements in healthy eating, symptoms of hypoglycemia, communication with physicians, depression, and reading food labels. Participants also had improvements in patient activation and self-efficacy.  

Who should participate:  Older adults with diabetes, pre-diabetes, and/or caregivers of those with diabetes.
