I understand that the Nelson Tennis Foundation and/or the owners of any tennis court used during the program shall have no responsibility to pay for medical treatment and related costs. I further understand and agree that the Nelson Tennis Foundation supplies any equipment used for tennis “as is” and that the Foundation disclaims all warranties, express or implied, including warranties of merchantability and fitness for a particular purpose. Knowing the risks described above, I agree, personally to assume all the risks and responsibilities surrounding my minor child’s use of the facilities and the equipment.
