Child must be able to care for themselves.Parent's Night OutEasterseals Northeast Indiana and McMillen Park Community Center are coming together to offer FREE care for individuals with special needs and their siblings while parents and caregivers enjoy a night out on a Saturday. You only need one ticket per family!Here’s how it works:•You bring your children ages 5+ (yes, adult children with special needs are welcome!) to McMillen Park Community Center. Siblings of those with disabilities are also able to come. Up to 15 siblings groups are able to attend.•We provide dinner (sponsored by Dominos), games, crafts and indoor playground time for them from 5pm-8pm.•You go out for a relaxing evening with peace of mind, knowing your family is cared for in a safe, fun environment.Waiver:In consideration of my and/or my child(ren)'s participation in any activities affiliated with the McMillen Park Community Center & Easterseals Northeast Indiana, without respect to location, I do hereby agree to hold free from any and all liability the McMillen Park Community Center & Easterseals Northeast Indiana and its representative officers, employees and members and do hereby for myself, my heirs, executors and administrators, waive, release and forever discharge any and all rights and claims for damages which I and/or my child(ren) may have or which may here-after accrue to me or my child(ren) arising out of or connected with our participation in any of the activities affiliated with the McMillen Park Community Center & Easterseals Northeast Indiana I hereby do declare myself and/or my child(ren) to be physically sound to participate in the activities per this event.The McMillen Park Community Center & Easterseals Northeast Indiana does not furnish accident/illness medical insurance. Medical bills, including prescription drugs will be the responsibility of the participant or their parent.I understand that registered sex offenders are not eligible to participate in or attend programs and I certify that I am not a registered sex offender. I understand the McMillen Park Community Center and Easterseals Northeast Indiana reserve the right to verify this fact against the Sex Offender Registry.I grant the McMillen Park Community Center & Easterseals Northeast Indiana the rights to take and use photographs and/or video of anyone included on this unit for print and electronic promotional material.If individuals are not picked up by 8pm the emergency contact will be called.
