Advance Directives Planning Event

We are excited to invite you to a free event on advance directives planning, led by Teri Ashley, a certified Advance Care Planning Specialist. Advance directives are essential legal documents that outline your medical care preferences if you're unable to communicate them yourself. This session will cover the importance of these documents, how to create them, and the different types available.

Teri, who is also a hospice volunteer, certified life coach, and restorative justice facilitator, will share her expertise and personal experience, inspired by her husband's passing in 2017. Her mission is to inform and empower you to make your own end-of-life choices.

Participants will receive essential resources, including:

• Five Wishes booklet
• Living Will
• Medical Durable Power of Attorney (MDPOA) form
• Conversation Starter Guide
• File for Life
