Teri, who is also a hospice volunteer, certified life coach, and restorative justice facilitator, will share her expertise and personal experience, inspired by her husband's passing in 2017. Her mission is to inform and empower you to make your own end-of-life choices.

• Medical Durable Power of Attorney (MDPOA) form

• File for Life
