Living Well with Chronic Conditions is designed to improve the self-management skills of people living with ongoing health issues. Conditions like diabetes, heart problems, mental health conditions, or chronic pain, can cause those affected to suffer health problems over many years. Originally developed and at Stanford University, this program teaches new strategies that will give participants the motivation, and skills needed to manage the challenges of living with a chronic health condition. Trained leaders will facilitate a six-week workshop that empowers those living
with ongoing health issues to learn the skills to increase their quality of life. The workshops are taught by two trained peer leaders, some who have health conditions themselves. Workshops are offered through the statewide Juniper network—a centralized hub
of community providers delivering evidence-based programs to help people manage chronic health conditions, improve balance and prevent falls, and foster well-being.
