Instructions: Entry form is attached below. Be sure you save the entry form before you fill it out, then
sign it and email it back to me at

1. Copy of front and inside of your scorebook
2. Copy of your current membership card*
3. Indicate if you have previously worked your dog on the Wild West Dog Sport field.
*Membership card and scorebook from an AWDF/FCI recognized organization.
If your dog has scorebooks in other AWDF/FCI recognized organizations, present them day of trial and we
will record trial results in all of them as required by FCI.

Two Tr./Ob./Pr. entries with the same dog: $125
