Integrated Care Operations, providing a detailed overview of “how to” operationalize the integrated care clinical model, including planning, staffing, facilities, scheduling, team-based care and workflow.
Navigating Common Challenges in Primary Care. Some challenges in integrated care are universal. This presentation provides an overview of potential solutions to common challenges in behaviorally enhanced primary care (lessons learned the hard way!).
Integrated Care Financing/Coding and Value-based Contracting. Discusses the financial sustainability of the integrated care clinical model.
