•2X2 PHOTO (new players)
•COPY OF BIRTH CERTIFICATE (new players)
•TRIMESTER 2 OR QUARTER 3 REPORT CARD (once available) ** MUST BE CORRECT MARKING PERIOD TO BE ACCEPTED
•2026 PARENT CONTRACT & CONSENT (online during registration)
•2026 YOUTH PARTICIPANT MEDICAL HISTORY FORM (online during registration)
