*DOB for ALL household members
*Income & Expenses for ALL household members
*3 Gift Wishes for each angel (NO gift wish valued over $50 each) 
*Accurate Clothing/Shoe Sizes for each angel

According to your zip code, approved applications will be assigned to one of the following Distribution locations: Clarksburg, Buckhannon, Gassaway, Weston or Elkins

*Approved families will receive an approval/appointment email with the distribution location & date. If you do not receive the email, you have not been approved. 
Christmas Food boxes will be provided to approved Angel Tree families in select counties only, you will receive notification in your approval/appointment email.
