Would you like for SDRC to pay for Club Beacons under Social Recreational activities?

For SDRC traditional consumers/clients:

1. Contact the consumer’s service coordinator to ask for approval for social recreation funding for Club Beacons.
2. Seek approval for 6 month intervals: $55/event twice a month for 6 months= $660

4. Once we receive this information, you will be given a Promo Code to register for each club event.

For Self Determination consumers/clients:

1. Add Beacons to your spending plan
2. Seek approval for a 1-year interval: $55/event up to 22 events a year
3. Once the consumer/client's revised spending plan is approved, please email us at ZmluYW5jZSB8IGJlYWNvbnNub3J0aGNvdW50eSAhIGNvbQ==. In your email, include the consumer/client's name, the guardian/conservator's name, their email, UCI#, service code, and instructions from the FMS to submit payment.
4. Once we receive this information, you will be given a Promo Code to register for each club event.

Cancelation/No Show Policy:

All ticket sales are final and are neither refundable nor transferable, unless cancelled with at least 48 hours notice.
