However, not all organizations have experienced teams dedicated to these types of investigations. As a result, other roles are often required to support or even lead investigations. In those situations, prior training and awareness will reduce the associated stress and increase the overall effectiveness of the process.TOPICSThis one-day seminar will use case studies and interactive discussions to increase the audience’s confidence and awareness of key concepts and tools associated with failure and incident investigations including:Language of investigationsAnatomy of an incidentStakeholders and investigation teamsInvestigation techniquesCausal factorsEvidence collection and preservationRole of lab work (e.g., material testing, metallography, fractography)Safety and site controlsReport writingRegulatory requirementsCorrective ActionsWitness statementsAttendees will leave the seminar with an increased level of confidence in their ability to support or lead investigations at their work site.Instructors:Megan Sutton, M.
