We will examine local system challenges and how we can work together across sectors to support people living with heart failure to get the best outcomes.The content of the study days will be the same, tailored to each region (Humber North Yorkshire, South Yorkshire and West Yorkshire). Each session will be supported by local heart failure experts who support people living with heart failure across the whole heart failure pathway.AimThe aim of this study day is to support healthcare professionals to develop their knowledge and skills to provide a person centred approach to recognise the signs and symptoms of heart failure, enabling a timelier diagnosis, swift escalation to the right treatment at the right time.Learning outcomesAt the end of the session, healthcare professionals will• develop their confidence in recognising the signs and symptoms of heart failure• be able to describe the appropriate referral of people with suspected heart failure• be able to summarise the management of different heart failure phenotypes• apply person centred evidence-based heart failure therapy to clinical scenarios• develop advance care planning consultation skillsStudy day dates and venue information:**Please note: if you are unable to join your region's study day, you are welcome to attend the study day in one of the other regions.
