**SPACE IS LIMITED--2 Cats Per Berks Colony and 1 Outside Berks--please complete form &amp; follow instructions

Microchip $5 extra.  
Ovaban Birth Control Available for $5 a bottle.

Bring a secure hard plastic carrier (NO ZIP TIES!)  for recovery with 2 towels.  

All items must be labeled with your LAST Name and Cat's Name!  

EXACT Cash Only due at time of admit.  
PICK UP SAME DAY at designated times between 11-2pm.
