This collaborative event brings together the Bridge Assistive Technology Program and South Sound Parent to Parent’s Peer Mentor, Information, and Support Program to help families learn about available assistive technology resources and receive one‑on‑one support with the application process.

Bridge Assistive Technology Program
Eligibility & Required Documentation:

To qualify for the Bridge Assistive Technology Program, documentation must be verified for each applicant’s residence, household income, and disability status. Documents are required for each qualifying individual.

Eligibility Requirements:
Applicants must meet all of the following criteria.

▪️Reside on Nisqually Indian Tribal Land or in Lewis, Mason, or Grays Harbor Counties.

▪️Have a documented disability
▪️Combined household income at or below 300% of the Federal Poverty Guidelines, or provide proof of financial hardship

Acceptable Documentation:
▪️Disability (one required)
▪️Doctor’s note including diagnosis
▪️MyChart summary including diagnosis
▪️After‑care summary including diagnosis
▪️Social Security Disability award letter

Income (for each household member earning income):
▪️Last year’s tax return
▪️Summary of government benefits
▪️Bank statement showing income deposits
▪️Social Security Disability award letter
▪️Recent pay stubs

Residence (one required):
Any document listed above that clearly shows current address.

▪️Driver’s license or government‑issued ID
▪️Utility bill

Important Note:
Social Security Disability documentation may satisfy all three eligibility requirements (disability, income, and residence).

Programa Bridge de Tecnología Asistiva. Elegibilidad y Documentación Requerida:

Para calificar para el Programa Bridge de Tecnología Asistiva, se debe verificar la documentación de residencia, ingresos del hogar y estado de discapacidad de cada solicitante. Se requieren documentos para cada persona que califique.

Requisitos de Elegibilidad:
Los solicitantes deben cumplir todos los siguientes criterios:

▪️ Residir en tierras de la Tribu Nisqually o en los condados de Lewis, Mason o Grays Harbor.
▪️ Tener una discapacidad documentada

Nota Importante:
La documentación de Discapacidad del Seguro Social puede cumplir con los tres requisitos de elegibilidad (discapacidad, ingresos y residencia).
